DATE OF ADMISSION :15-10-2022. C/o :  Shortness of breadth and loin pain since 2 months. HOPI: 65 year old man, farmer by occupation, came to the casualty with complaints of Shortness of breadth and loin pain since 2 months. Patient was apparently asymptomatic and had a good appetite before. He slowly developed shortness of breadth and severe knee pains from 2 months  Patient was diagnosed and told that he had high creatinine levels due to which he had to undergo dialysis. He was asked to get admitted in the hospital to undergo dialysis. Patient is admitted in the hospital on  15-10-2022.  He has undergone 4 sessions of dialysis. Before undergoing dialysis on 18-02-2022 his creatinine levels were  7.4mg/dl. Creatinine levels after 3 sessions of dialysis is  3.6mg/dl. After the first session of dialysis he developed a black pigmentation on right hand. Past History :  No similar complaints in the past. 25 yr old student with fever and generalized weakness Date of admission: 15-9-22. 25 years old Student with fever and generalized weakness. 25 years old male patient who is preparing for govt. job is also an agriculture worker , came to the opd on 15.9.22  with the complaints of  •fever since 15 days  •generalized weakness since 1 month. HOPI. patient was apparently asymptomatic 1 month back,then he developed generalized weakness and dizziness. He visited doctor for the same complaints,was given some medications and was advised to visit a hospital. • h/o fever since 15 days. On and off.  Low grade. Not a/w chills, rigor. No h/o muscle pain. H/o loss of weight of 10 kgs in 6 months  He does agricultural work for 6 hours in a day and then he prepares for govt. job at night. H/o decreased intake of food and inadequate sleep since 1 month.
