65 year old male with shortness of breadth and loin pain.
DATE OF ADMISSION:15-10-2022.
C/o: Shortness of breadth and loin pain since 2 months.
HOPI:
65 year old man, farmer by occupation, came to the casualty with complaints of Shortness of breadth and loin pain since 2 months.
Patient was apparently asymptomatic and had a good appetite before.
He slowly developed shortness of breadth and severe knee pains from 2 months
Patient was diagnosed and told that he had high creatinine levels due to which he had to undergo dialysis.
He was asked to get admitted in the hospital to undergo dialysis.
Patient is admitted in the hospital on
15-10-2022.
He has undergone 4 sessions of dialysis.
Before undergoing dialysis on 18-02-2022 his creatinine levels were 7.4mg/dl.
Creatinine levels after 3 sessions of dialysis is 3.6mg/dl.
After the first session of dialysis he developed a black pigmentation on right hand.
Past History: No similar complaints in the past.

Bowel and Bladder movements- Regular
Addictions- None.
Diet- Mixed.
Allergies-No allergies.
FAMILY HISTORY: Patients mother hand knee pains(arthritis) , Diabetes Mellitus.

PR- 96 bpm
BP- 140/90 mmHg
RR- 16 cpm
SPO2- 98% at RA
