38 yr old female with severe pain in the RIGHT LOWER ABDOMEN.

38yr old female complaints of severe stomach ache in the right lower abdomen.

DATE OF ADMISSION:18-02-2022.

C/o: Severe stomach ache in the right lower abdomen since 3 days.
HOPI:
38 year old female, saleswomen by occupation, came to the casualty with complaints of severe stomachpain in the right lower abdomen.
Patient was apparently asymptomatic
She slowly developed mild stomach ache since 3 days in the right lower abdomen,in the umbilical region, which she thought was common as she goes to her workplace by walk daily.
But as it became severe she came for a checkup got diagnosed and told by the doctors that there was a inflammation seen in the appendix region and it was acute appendicitis.
She was told that it was very severe and had to undergo an operation immediately.
She was asked to get admitted in the hospital to undergo surgery.
Patient is admitted in the hospital on
 8-03-2022.
She has undergone laparoscopic appendectomy on 8-03-2022  ,between  9:00 pm to 10:00 pm.
She had white discharge and burning sensation while micturation before undergoing the surgery.

Past History: No similar complaints in the past. 
But she used to experience mild stomach ache while walking speedly and climbing stairs.
Diabetes mellitus since 1 year.

PERSONAL HISTORY:
Appetite- Normal
Sleep- adequate
Bowel and Bladder movements- Regular
(Burning micturation).
Addictions- No addictions.
Diet- Mixed.
Allergies-No allergies.

FAMILY HISTORY: Insignificant.

VITALS:
Temp- 98.4 F
PR- 86 bpm
BP- 110/80 mmHg 
RR- 16 cpm
SPO2- 98% at RA

GENERAL EXAMINATION:
Patient is conscious, coherent and cooperative. 
Pallor: No
Icterus: No
Cyanosis: No
Clubbing: No
Lymphadenopathy:No
Edema: No

INVESTIGATIONS ON: 10-03-2022.


Treatment:
Inj Taxim 1g/ iv/BD
Inj Metrogyl 100ml/iv/BD
Tab pan 40mg/PO/OP
Tab Hifenac P/PO/TID
Tab vit -c 500mg/PO/OD.

17 year old male complains of fever and legs pain.

DATE OF ADMISSION :22-10-2022. C/o :  Severe fever and legs pain since 4 days. HOPI:  17 year old boy,  fast food centre master by occupation, came to the casualty with complaints of severe fever and legs pain since 4 days. Patient was apparently asymptomatic and had a good appetite before. He slowly developed fever,legs pain since 4 days, abdominal pain since 1 day, vomitings 6-7 episodes/day, bilious loose stools 4-5 episodes/day( liquid consistently). Patient was diagnosed and told that he had high creatinine levels due to which he had to undergo dialysis. He was asked to get admitted in the hospital to undergo dialysis. Patient is admitted in the hospital on  22-10-2022. He has undergone his 1st session of dialysis on  29-10-2022. Hence, he has undergone 1 session of dialysis.
