62 year male with loss of appetite.

62 YEAR MALE COMPLAINTS OF LOSS OF APPETITE.

DATE OF ADMISSION:18-02-2022.

C/o: loss of appetite since 10 days.

HOPI:
62 year old man, farmer by occupation, came to the casualty with complaints of loss of appetite.
Patient was apparently asymptomatic and had a good appetite before.
He slowly developed fever,loss of appetite for 10-20 days.
Patient was diagnosed and told that he had high creatinine levels due to which he had to undergo dialysis.
He was asked to get admitted in the hospital to undergo dialysis.
Patient is admitted in the hospital on 
18-02-2022.
He has undergone his 1st session of dialysis on 22-02-2022.
 Then immediately 2nd session of dialysis on 23-02-2022, and
3rd session of dialysis after three days that is on 27-02-2022.
Hence, he has undergone 3 sessions of dialysis.
Before undergoing dialysis on 18-02-2022 his creatinine levels were 5.2mg/dl.
Creatinine levels after 3 sessions of dialysis is 3.6mg/dl.
After the second session of dialysis he developed a small swelling(cyst) on his left foot.
After the third session of dialysis the patient suddenly developed shortness of breath and was and unable to speak.
Then the patient was immediately shifted to ICU for further treatment on 
01-03-2022.
Then after the treatment he was discharged on 04-03-2022.

Past History: No similar complaints in the past.
Hypertension, Diabetes mellitus.

PERSONAL HISTORY:
Appetite- loss of appetite
Sleep- Inadequate
Bowel and Bladder movements- Regular
Addictions- Patient smokes beedi.
He does not consume alcohol .
Diet- Mixed.
Allergies-No allergies.

FAMILY HISTORY: Insignificant.

VITALS:
Temp- 98.4 F
PR- 90 bpm
BP- 140/80 mmHg 
RR- 22 cpm
SPO2- 98% at RA

GENERAL EXAMINATION
Pallor: No
Icterus: No
Cyanosis: No
Clubbing: No
Lymphadenopathy:No
Edema: No

INVESTIGATIONS ON: 26-02-2022 
RFT-18-02-2022
RFT- 27-02-2022

DIAGNOSIS: Impaired kidney function.

TREATMENT:
Inj LASIX 40mg IV/BD
Inj PAN 40mg IV/OD
Ing IOFER 4mg IV/OD
T NODOSIS 500mg PO/BD
T OROFER XT PO/OD.

17 year old male complains of fever and legs pain.

DATE OF ADMISSION :22-10-2022. C/o :  Severe fever and legs pain since 4 days. HOPI:  17 year old boy,  fast food centre master by occupation, came to the casualty with complaints of severe fever and legs pain since 4 days. Patient was apparently asymptomatic and had a good appetite before. He slowly developed fever,legs pain since 4 days, abdominal pain since 1 day, vomitings 6-7 episodes/day, bilious loose stools 4-5 episodes/day( liquid consistently). Patient was diagnosed and told that he had high creatinine levels due to which he had to undergo dialysis. He was asked to get admitted in the hospital to undergo dialysis.
